Spontaneous Plans

I'm booked on a super early (like getting up at 3am) flight tomorrow from Seattle to location X where Bean currently is.  I don't know if it will be too late.  I did my trigger Thursday night at 10pm so by the time I land it will be 60 hours after trigger.  


My temp was higher this morning but this was on very little sleep and taking the temp hours earlier since I had to be up to catch the ferry to Seattle.

I was so upset when I talked to Bean.  He got me on the earliest flight tomorrow, and then I fly home the following day. 

Not sure how realistic this plan is.  But I know I have a higher percentage doing this verses the IUI.

A Move Closer to IVF

Today's IUI was done with 1.3 million sperm, not good.  "Vial 1" from May had 10 million post freeze, and most of those didn't survive the thaw.  I asked about "Vial 2" from June that was still frozen and they said it would produce about 2 million, 3 at the most.  A little bit better.  But the RE would like 10 million and above.  Plus motility and R&L went down.

They gave me the option of canceling the cycle and just walking away.  But I already did the Femara, trigger shot, had two great eggs and got lost in Seattle trying to get to the RE.  I already drank four cups of water to have a full bladder and I was half naked on their table.  The chances of getting PG with those numbers are next to nothing, even with two good quality eggs.  But I paid the $225 for the IUI and went through with it.  The fact that I actually had eggs to ovulate was rare and I just can't go home to have timed intercourse as a backup.  I had to do something.  

This is my first and last cycle doing IUI from a non fresh sample.  "Vial 2" will be labeled for IVF only, or discarded in the future.  

I will wait for AF and then next cycle I will have no meds except Metformin.  The October IUI will be canceled obviously.  My PCOS will probably just make my ovaries go into sleep mode again.  Right before my husband comes home from deployment I will take Provera to start my cycle again.  

I'm going to call the RE on Monday and double check where my spot is on the IVF wait list.  Then possibly having a WTF appointment and talk about having my ovaries drilled out while I wait in October.

BBT is looking somewhat normal.  The reading come out better when I do them vaginally, not orally.  Is that an implantation dip on 7dpo?  Probably just wishful thinking.

I'm half way through my two week wait.  It's too early to tell if it worked or not.

Hormonally, I am pregnant.  Thanks to the trigger shot, which contains hCG hormone, same found in pregnancy.  I have been testing out my shot and right about now (8dpt) it should leave my system.  But my HPT do still have a faint line.  If I'm PG then: A. the line goes away and then comes back or B. the line never goes away but keeps getting darker.  I'm sure some of my symptoms are coming from the hCG injection.

Some things have been happening with my family that have been keeping me busy.  Nothing bad, just stuff that I can't post here for a little bit.  

Do I think I'm pregnant?

Yes - It's IUI, the sperm and egg are perfectly timed.  How could one not be PG?  I know a few people that got PG with their first IUI.  My boobs are not sore.  Nibbles a little perky, but boobs as a whole, nothing.  My boobs are always sore within a couple days of O.  Except the month I got my BFP, sore boobs didn't start 'til 11 or 12dpo.  This happened to my friend too.  Sore banger every month, except the month she got PG.  I was dizzy at the store tonight, really dizzy.  Maybe it happened a few other times too, but not as bad.  And lots of cramping down there.  It's feels like AF is coming and I even went running to the bathroom on the ferry the other day because I was so wet down there.  Today there is cramping too but it feels a little different, and more towards the right side.  I had a luteal phase dip in my BBT this morning.  I am pretty tired also.

No - You know I'm really not a lucky person.  The timing is too right in our life.  Sometimes people have to do IUI after IUI.  My acne is bad too.

First IUI

Bean had his appointment first yesterday morning.  They need to wash the sperm before inserting it for the IUI.  Washing it means removing any dead ones and separating the actual swimmers from the seminal fluid.  If you insert the fluid part into the uterus it can cause cramping and even for it to collapse.  Normally the cervix filters everything out but they bypass that with an IUI.

The SA came out alright, I think.  I'm really bad about reading SA numbers because "good" or "bad" doesn't depend on just one number, many things go into it.  I don't think these numbers are amazing.  But each one is above what the RE likes to see it at.  If it is too low then they'll cancel my IUI.

While the crew was getting their bath Bean and I wasted time by having lunch and checking out the local mall with the Apple store.  I needed to scope out where I'm standing in line for the new iPhone 5.

The rooms at this RE are nice, and the office is huge.  I don't normal go to Tacoma, usually we are at Seattle or the Army hospital.  Everything went pretty quick.  They brought me back in the room and had me undress.  The nurse double checked Bean's info and my bracelet to make sure I got my husband's sperm.  You know, because that it kind of a big deal if I get someone else's.

The RE put the spectrum inside me without any lube because that kills the sperm.  It didn't hurt since there is cervical mucus in there right now.  It took a few minutes to get the catheter up past my cervix.  It was really crampy.  She kept asking if I was cramping and wanted adjust the catheter or spectrum if it was.  At first I told her it was, but then realized the whole procedure would last longer with her trying to fix it.  I'd rather tolerate some pain for a minute or two then have you keep adjusting everything and drawing it out.  I didn't feel the sperm being injected and it was quick only took about 30 seconds.  After that everything was removed.  I laided on the bed for five minutes with the cramping continuing before I got up.  I was surprised nothing came out like after sex.  Bean went to pay and then drove me home.  I spent the rest of the afternoon sleeping since I got such little sleep the night before with the excitement.  

I had my IUI 28 hours after my trigger and about 38 hours after my +OPK.  Ovulation usually occurs 24-48 hours after ether of those.  I'm not sure when I actually ovulated since I didn't feel the pain, but I'm pretty sure it was the day of my IUI.  We came home and got a little extra action in, just in case.

They told me I can start testing on August 24th (15dpo, 2 weeks & 1 day) which I highly doubt I'll be waiting that long.  I'm just not a patient person.  But then again maybe I will because my due date for my last miscarriage is the 25th.  I'll ether be super happy or crying all day.  I am testing out my trigger though.  I think it is normal in ones body for 7-9 days.

Trigger Shot

I did my first injection yesterday.  It was into my stomach but really wasn't as bad as it sounds.

I took Ovidrel and it already comes in a prefilled syringe.  I think the cost is between $50-100 without insurance, mine was covered though.  It has to be refrigerated.  The shot is an injection of hCG hormone, same type found in pregnancy.  So you turn pregnancy tests positive for a week or so after. 
  1. I woke up at 8am to get the meds out of my fridge and put on my counter.  It's good to have them at room temperature when it is injected.  Fell back asleep.
  2. At 9am I woke back up to do the injection.  Since Ovidrel is prefilled there is no mixing or measuring like other fertility medication.  You just take it out of the package and the needle is already full of medication.  Take the cap off.
  3. You know when on those doctor shows the actor flicks the needle a few times and they a tiny bit of fluid comes out the top?  It's to get the air to come out of the syringe.  Injecting someone with air can be fatal.  This is common sense to me but I know it is not to everyone.  Plus I've spent years filling syringes for my sick ferrets.  Even though everything is oral for them it is sometimes easier to get a better reading of how much medication is in the syringe without it part filled with air.   Hold the syringe with the needle pointing up so the air bubbles float to the top.  Flick it a few times so any lingering bubbles make it to the top also.  Then press on the plunger 'til all the air gets out and a little bit of liquid meds come out.  You want to make sure all the air is out but not lose much of the meds.  I probably went overboard but I'm more afraid of dying from air embolism than anything else.
  4. Find a spot where you want to do the injection.  I was told on the belly about an inch or two below or to the side of my belly button.  I think you can also do your thigh or butt also.  But my legs are really muscular and I can't reach my own ass.  My stomach is good enough for me.  Wipe the spot with an alcohol swab.  Also wash your hands before you do this, and don't ever touch the needle.  You don't need to be injecting germs too.
  5. I grabbed a fat roll or some skin (easy for some of us) and inserted the needle as far as it would go.  The length of the needle isn't too bad, only an inch or 2cm.  Then let go of the skin and slowly pushed on the plunger 'til everything was out.  Slowly pull the needle back out once it is empty.  
  6. My directions told me to put gauze over the site but I didn't have any so I just put a Hello Kitty band-aid over it.  Not needed though.
  7. Put the used syringe in a sharps container.  My RE told me they don't give out the actual container but (depending on where you live) I could put all my needles in a plastic soda bottle and then drop them off at a designated location.
I had no pain from this.  Even took a photo after the meds were injected and needle was still in me.  I've probably been through more medical procedures, surgery and testing than most women my age.  Not much is painful to me, or even worries me anymore.  One thing I learned a long time ago before infertility treatment and when I was dealing with cervical dysplasia is to believe that whatever you are going through is "normal".  And everyone else is a bunch of wimps.  This works better for me than seating around feeling sorry for myself.

My IUI is scheduled for today.  Bean has his appointment at 10:30 to make a deposit.  This will be our last fresh cycle for awhile.  And then I'm scheduled at 1:15 for the IUI.

Ultrasound 8/6

I went to my CD15 ultrasound today.  The RE had a trainee with him today so he spent extra long with the U/S.  At first they couldn't find my uterus.  A shadow or something was in there.  I assured the RE it was still there.  (Gosh I'm going to turn into one of those.)  After five minutes or so and a lot of wand action he found it.  My lining was 8mm this time! Perfect for implantation.  Left ovary is doing nothing.  But the follicle on the right ovary is now 17.5mm!  

The plan:  I'll do OPKs all today, then tomorrow, then one Wednesday morning.  They are waiting for the natural LH surge.  If none come up positive then I'll trigger.  Which looks like a great possibly since my OPKs are still really light.  They showed me how and where to inject it, along with needle disposal.   Then I went to wait in a 45 minute line at the pharmacy to pick up the Ovidrel.

The only thing that seems odd is they told me to trigger on Wednesday morning or afternoon, but I've only ever heard of people triggering at night.  I'm going to call up the place doing my IUI tomorrow just to double check.

RE Appointment 5/24

My appointment went good today.  We did get lost when we first arrived.  I asked the little Korean lady at the information desk where to go but she didn't know where the "fertility clinic" or "Reproductive Endocrinologist" was.  She gave me two options.  First being the OBGYN on the third floor, for pregnancy tests, pregnancy and giving birth.  No, that's not me.  Plus when I made my appointment I was told me ground floor or lower level.  The second option was the OBGYN on the ground floor.   Hmm, I don't think that is me, but it could be?  I walk down to the OBGYN clinic and as soon as I walked in the door I notice the waiting room full and there are tons of newborns and babies everywhere.  Fuck!  I really hope this isn't the place.  Sure enough he sends me a door down to the actual fertility clinic.  This waiting room just had a couple waiting alone and a few women.  Much more my taste.

  • Medical History - They didn't seem to care about the records that have yet to arrive.  Just asked me questions.
  • Ultrasound
    • Uterine Lining - Measured 15mm, too thick.  Obviously it's been some time since AF.
    • Ovaries - I've never had cysts before, at least they haven't been found.  She showed me the left ovary, covered in small cysts.  Then scanned to the right ovary, also covered in cysts.  It's nickname is Ring of Pearls.  Yup, the classic PCOS cysts.
  • Blood Work - I go for that tomorrow morning.  I'm having glucose done along with a few other thing, mostly stuff I've had done in the past.
  • Semen Analysis - She's a little worried about Bean's low results in the past.  I'm not too concerned, they could be worse & I seen PG with less.  They are not bad enough to send us to an Urologist.

Dr. C told me there is overweight PCOS and skinny PCOS.  I have skinny PCOS.  Even with my anovulation and cysts I still don't look PCOS.  Which is probably why I've made it so long without a diagnosis.  I'm not overweight enough and I don't have thinning hair, hair in strange places or darkening of the skin.  She also said no amount of weight loss will make my PCOS better.  Though I don't like to make excuses nor am I lazy, this is something I have thought about from time to time due to the fact that my problems began when I was 16 and underweight at the time.  I actually wasn't allowed to donate blood in high school because I didn't weigh enough.  If I see the same problems at a BMI of 18 and a BMI of 29, weight probably it's related to it.  

Cervix

The description of the my cervix from the RE looks bad.  I guess most of it is gone and it's very short.  I am at a great risk for cervical incompetence and a cervical cerclage is most likely going to be needed.  This also comes with bed rest.  This scares me because I am a military wife without family in the area and very few friends.  I will have to figure out a plan once I get pregnant.  This also isn't new news to me so no surprise about the cerclage.  

She is having me met with the high risk OBGYN even before pregnancy is achieved so I know what to expect.

Overall

Even with my failing cervix, my polycystic ovaries and Bean's 23 million sperm count we are ready to move forward with IUI.

Move to IUI

I began spotting this morning, randomly without any cramping.  Not sure if this is AF, but if it is then tomorrow or the following day should be CD1.

I need to have a beta PG test done to make sure I'm not PG.  This part really doesn't seem necessary.

On CD3 I'll start Clomid 50mg for five days.  I'm not a fan of Clomid, I didn't ovulate at 150mg, let along 50mg.  My five rounds of Clomid before were unmonitored they don't know how I responded to it, other than not releasing eggs.  This time I will have ultrasounds to show the number of eggs and their sizes, along with the trigger shot to manually release them when they are ready (vs waiting for if/when my body to do it).  So I guess it's better, but I still don't like Clomid.  I've just already been down the road before and see no hope there.  

Around CD12 I'll have my first ultrasound to check the size of my follicles.  Possibly a few ultrasounds after that until my follicles are large enough.

I'll be given the Ovidrel trigger shot.

Following that will be my IUI with Bean's fresh or frozen sperm, depending on if he is home or not.  If it is fresh sperm and Monday-Friday then I'll go to Tacoma, if it's frozen or fresh on Saturday or Sunday then I'll go to Seattle.  

Next Tuesday we drive to Seattle to "drop off" some sperm to be frozen. 

Cost 

Clomid & Ovidrel are covered by my insurance, along with any blood work or ultrasounds.

The IUI is $200 for the sperm washing and inseminating me.

Freezing sperm and keeping it frozen is $250 for the first three months, then $125 every three months after that.  

This is a pretty good deal.  There is about a 20-25% chance of it working each cycle.

I'm also on the list for IVF.  It's about a year wait and costs $6000.

Behind the Glass

I think I'm going to have to be induced with Provera again.  It's a little upsetting.  The side effects are not the bad for me.  It's frustrating that I have to keep coming back to it.  I took it last April, then July, August and March.  I have extra here that I could take right now, but I'm saving it and holding off.  With my next RE appointment two weeks away they'll probably want to run more tests on me.  If I take the Provera now and it works quickly (probably not) then I could miss the specific testing days and have to wait another cycle for testing.  I'll just tough it out.  I'm about CD41 today and my cervix goes hard to soft over and over again.  My menstrual cramps are gone.  It seems so odd to have them so strong, then nothing.

I've been looking online for information about this doctor's office.  They seem to like to start women on Clomid before going to injectables, even if she has had no success on it before.  I did five rounds of Clomid last year, however I was never monitored via ultrasound.  Because there was no monitoring there was also no trigger shot.  With my past of not ovulated on Clomid 150mg, Dr. H wanted to get rid of Clomid all together and move me to Gonal-f (injectable) and Ovidrel (trigger).  I'm not sure that the new doctor will have the same opinion.  Clomid is not the worst thing in the world.  The side effects are not ideal, and it's not a needle.  It's just kind of territory that was already covered.  

I had a dream the other night about not being able to get the needles into my skin.  Ether my skin was leather or the needle was dull.  I just kept stabling myself over and over again in the stomach.

Attempt at Running

I went running today for the first time even pretty much.  I had a gym membership back in NH.  It's a chain gym so they are all over the US, except here.  I tried looking into a different gym here but so far the only one I like is double the price I was paying before.  This gym does have classes and other benefits that my old one didn't so I'm sure it's worth the extra money.  I just haven't come to terms with spending it yet.  I'm told I can get a free membership to the YMCA but it's a few towns over.  And an extra 10 minutes of driving time is probably going to used as an excuse for not going at all.  

I decided since I am new here and haven't seen much of the town that I'd just walk around and see it all by foot.  Which lead to running.  I'm not much of a runner.  I refer to myself as the fat kid.  I'm slow and have no coordination.  I'll see where it takes me.  My back and legs already hurt so that's good.

Still Standing

A couple people in the last few days have directed me over to Still Standing Magazine.  It's a new website launched this month about infertility and loss (pregnancy or infant).  I added myself to their monthly blog list.  They don't have too much on there yet but it does have promise. 

Behind the Glass

I did see this article, No Mother’s Day For Me, on Still Standing that mentioned Mother's Day is like sitting outside a candy store and watching everyone inside have fun.

This is interesting because I always describe it as similar.  I feel like I'm in grade school and everyone is allowed to go out for recess, but not me.  I got in trouble or didn't do my homework or something.  I have to stay inside.  Sitting next to the window I look out all my friends running around and having fun.  I wish I could be out there with them except I did something wrong and have to complete my punishment.
  
Only thing now I'm all grown up.  I'm infertile and still standing behind the window watching everyone have fun, smiling.  They are all so happy.  I can't figure out what I must have done wrong this time, what the cause of my punishment is.  How I can make it up?

I'm stuck behind the glass.  I can't find a way to the other side.  I sometimes pound on it, very few people hear me.  I am alone with tears running down my face.  Every once and awhile there is some hope, but it doesn't stay for long.

I feel like everyone I know is given something I want so much so easily.  It's just handed to them with little or no effort, sometimes they didn't even want it to begin with.  They go along with their pregnancies with little or no complications.  Their baby is perfectly healthy.   It's not like the car we want with all the features.  Or the new TV.  The want is something different.  We try and try and try, over and over again with so much hope.  Month after month, year after year.  Just hoping if this herbal tea, different drug or break from it all together works.  Then a miracle happens.  Only to find out the heart stopped, we go into labour early or the our baby is born stillborn.  We are left with nothing again.

This Mother's Day I will be behind the glass just like many other women struggling with infertility and the loss the their baby.  I will be alone and probably will not talk to anyone that day.  I will not be getting a handmade card, flower or chocolates.  Or even be getting a "Happy Mother's Day-to Be".  My child is no longer alive.  I would have been 25 weeks, 1 day on Sunday.  Any effort I have ever made at being a mother means nothing.  I will be forgotten, my baby will be forgotten.

Post-Op & Planning Appointment 11/3

The Surgery:
It went well.  I recovered quickly and he didn't even look at my stomach at the appointment.  I guess the stitches (?) is suposed to fall off or go away on it own.  I've never actually had stitches before.

RE talked about the same things from the pictures before.  

Right Fallopian Tube:  Eh but he's not too concerned the risk of an ectopic pregnancy.

Uterus:  I have an hyperplastic endometrium.  Not too much of a concern though.  The biopsy of it came back normal.

Cervix:  I also have cervical stenosis as "very little normal cervix was remaining" and it was a bitch to dilate.  This was already assumed for awhile.  My cervix is scarring over and the opening is very narrow.  This is why cramps are so painful to me and my HSG was a trip from hell.  It's complements of my LEEP in 2005 and 2009, but mainly the second one.  It's a rare risk of having a LEEP done but of course my body loves to be "rare".  I forgot to ask Dr. H about the possibility of having an cervical cerclage put in if I do get PG.  The actual thought of having my cervix stitched closed terrifies me so we will put that thought off for awhile.  FYI a cervical cerclage is used in women that have a incompetence.  The cervix begin to dilate way before it should, maybe times due to the weight of the growing baby.  It results in miscarriage or early labor.  The cervix is stitched closed around 14 weeks and the stitches are removed around 37 weeks.

Ovaries, left fallopian tube and vagina are all good.  Fuck something semi works.  

What's Next?:

I have CD21 Progesterone BW next Monday.  I need to have at least a 3.3 to have ovulated, even if it's a crappy ovulation.  No 1.7!  A 10 is ideal for a non-medicated cycle.  (It was 15 when I was Clomid or if I'm on any other drugs.)

And then I need to decide, ovulation or not.  No ovulation then Provera will be my buddy again.

Dr. H began talking about doing an IUI (intrauterine insemination).  It's a couple types of shots and then Bean's sperm is poured into my uterus once I've ovulated.  My cervix hates me and I fear having another tube stuck up it, even though the RE assures me it is much smaller than the one for the HSG.  The cost is between $2000-2500 which will not include my meds, an additional $500.  But spending $3000 does not 100% get me a baby, only about a 17% of one.  So odds are I'll need multiple which is $3000 each cycle/month.  

And then there is IVF.  We are not even going to get into that because each cycle costs $12,000.  Again no 100% guarantee.  But if I buy two (results in BFN) then the third if free.  How nice of them!  If I spend $24k I'd better get a lot for free, along with a free crib, stroller and all the other stuff.  We can just pretend like this doesn't exist for now, like the cerclage I'll probably need.

My insurance will NOT cover any of my meds used for an IUI or IVF.  In fact they will even refuse to pay for basic U/S and BW.  They want no part of it.  (Then again my infertile ass is expensive, I don't want to pay for me ether.)  But they will cover my meds for timed intercourse (TI).

So for TI it is the same meds and monitoring as an IUI.  Except when I ovulate I have sex like a normal person, you know penis in the vagina, no tube of sperm in the uterus.  I'd like it to be as normal as possible.  This has a lower success rate but then again it's a lot cheaper.  

The meds I'd use for the TI (or IUI) are:

Gonal-f RFF Pen 450 IU @ 75iu/day - Daily injection used for ovulation induction (OI) with FSH hormone from CD4 for a week or two.  This makes my ovaries produce lot 'o eggs.  This is much like Clomid but for those who don't do so well on it.  I can't wait to stab myself in my stomach!

Ovidrel - Once injection of HCG hormone for my ovaries to drop all the eggs.  I'll be monitored with several U/S to see when my eggs have fully matured with Gonal, then this shot is used.  

I might be on Progesterone also to reduce the risk of MC since my body doesn't produce enough of it on its own.

The injectables come with a higher risk of multiples.  I'm pretty sure it made Bean worry a bit when the nurse said that.  But it's a 10% chance of twin, and much lower for triples and so on.  She let him know there wouldn't be six which I'm sure he was picturing in his head.  To be honest we are both pretty excited for twinkies!

Decision:

I'm going to see what the Progesterone comes back as.  If it's good then I'll hang out on Vitex and hope to be normal for awhile longer.  Or I'll move to TI with the injectables.  If it's too low then I'll definitely be moving to TI.

I'm not ready for IUI.  We have the money but I feel I haven't had enough time having natural sex to have given it a good shot.  I've been having unprotected sex for over a year but very rarely have my ovaries actually released eggs.  And a couple my age only has 20% change each time with perfect timing.  

I'm also waiting to move since our new home (hopefully) has an Army base that does IVF at a discounted rate and includes the drugs.  

Bean is most likely going to be sent back out to sea in the spring.  I can do IUI and IVF while he is gone with his frozen sperm.  I'm going to use the "fresh" stuff now.

If TI works:

I'll have BW for check my betas (PG test) 14dpo after each TI cycle.

I will stay at my RE until 8-14 weeks before being sent to an OBGYN.  My first fetal U/S will be at 6 weeks, then one or two more at a later date before going to the OBGYN.
